2025-10-15mm: prevent poison consumption when splitting THPQiuxu Zhuo1-1/+2
When performing memory error injection on a THP (Transparent Huge Page) mapped to userspace on an x86 server, the kernel panics with the following trace. The splitting process employs a mechanism, implemented in try_to_map_unused_to_zeropage(), which reads the pages in the THP to identify zero-filled pages. However, reading the pages in the THP results in a second in-kernel #MC, occurring before the initial memory_failure() completes, ultimately leading to a kernel panic. See the kernel panic call trace on the two #MCs. [5] Re-access pages in the hw-poisoned THP in the kernel. [6] Triggered in-kernel, leading to a panic kernel. In Step[2], memory_failure() sets the poisoned flag on the page in the THP by TestSetPageHWPoison() before calling try_to_split_thp_page(). As suggested by David Hildenbrand, fix this panic by not accessing to the poisoned page in the THP during zeropage identification, while continuing to scan unaffected pages in the THP for possible zeropage mapping. This prevents a second in-kernel #MC that would cause kernel panic in Step[4]. Thanks to Andrew Zaborowski for his initial work on fixing this issue. 2025-10-07mm/rmap: fix soft-dirty and uffd-wp bit loss when remapping zero-filled mTHP ↵Lance Yang1-5/+10
subpage to shared zeropage When splitting an mTHP and replacing a zero-filled subpage with the shared zeropage, try_to_map_unused_to_zeropage() currently drops several important PTE bits. For userspace tools like CRIU, which rely on the soft-dirty mechanism for incremental snapshots, losing the soft-dirty bit means modified pages are missed, leading to inconsistent memory state after restore. As pointed out by David, the more critical uffd-wp bit is also dropped. This breaks the userfaultfd write-protection mechanism, causing writes to be silently missed by monitoring applications, which can lead to data corruption. Preserve both the soft-dirty and uffd-wp bits from the old PTE when creating the new zeropage mapping to ensure they are correctly tracked. 2025-10-07mm/thp: fix MTE tag mismatch when replacing zero-filled subpagesLance Yang1-7/+1
When both THP and MTE are enabled, splitting a THP and replacing its zero-filled subpages with the shared zeropage can cause MTE tag mismatch faults in userspace. Remapping zero-filled subpages to the shared zeropage is unsafe, as the zeropage has a fixed tag of zero, which may not match the tag expected by the userspace pointer. KSM already avoids this problem by using memcmp_pages(), which on arm64 intentionally reports MTE-tagged pages as non-identical to prevent unsafe merging. As suggested by David[1], this patch adopts the same pattern, replacing the memchr_inv() byte-level check with a call to pages_identical(). This leverages existing architecture-specific logic to determine if a page is truly identical to the shared zeropage. Having both the THP shrinker and KSM rely on pages_identical() makes the design more future-proof, IMO. Instead of handling quirks in generic code, we just let the architecture decide what makes two pages identical. 2025-09-21mm, swap: use the swap table for the swap cache and switch APIKairui Song1-4/+15
Introduce basic swap table infrastructures, which are now just a fixed-sized flat array inside each swap cluster, with access wrappers. Each cluster contains a swap table of 512 entries. Each table entry is an opaque atomic long. It could be in 3 types: a shadow type (XA_VALUE), a folio type (pointer), or NULL. In this first step, it only supports storing a folio or shadow, and it is a drop-in replacement for the current swap cache. Convert all swap cache users to use the new sets of APIs. Chris Li has been suggesting using a new infrastructure for swap cache for better performance, and that idea combined well with the swap table as the new backing structure. Now the lock contention range is reduced to 2M clusters, which is much smaller than the 64M address_space. And we can also drop the multiple address_space design. All the internal works are done with swap_cache_get_* helpers. Swap cache lookup is still lock-less like before, and the helper's contexts are same with original swap cache helpers. They still require a pin on the swap device to prevent the backing data from being freed. Swap cache updates are now protected by the swap cluster lock instead of the XArray lock. This is mostly handled internally, but new __swap_cache_* helpers require the caller to lock the cluster. So, a few new cluster access and locking helpers are also introduced. A fully cluster-based unified swap table can be implemented on top of this to take care of all count tracking and synchronization work, with dynamic allocation. It should reduce the memory usage while making the performance even better. 2025-09-21mm, swap: wrap swap cache replacement with a helperKairui Song1-8/+3
There are currently three swap cache users that are trying to replace an existing folio with a new one: huge memory splitting, migration, and shmem replacement. What they are doing is quite similar. Introduce a common helper for this. In later commits, this can be easily switched to use the swap table by updating this helper. The newly added helper also makes the swap cache API better defined, and make debugging easier by adding a few more debug checks. Migration and shmem replace are meant to clone the folio, including content, swap entry value, and flags. And splitting will adjust each sub folio's swap entry according to order, which could be non-uniform in the future. So document it clearly that it's the caller's responsibility to set up the new folio's swap entries and flags before calling the helper. The helper will just follow the new folio's entry value. This also prepares for replacing high-order folios in the swap cache. Currently, only splitting to order 0 is allowed for swap cache folios. Using the new helper, we can handle high-order folio splitting better. 2025-09-13mm/migrate: remove MIGRATEPAGE_UNMAPDavid Hildenbrand1-23/+22
migrate_folio_unmap() is the only user of MIGRATEPAGE_UNMAP. We want to remove MIGRATEPAGE_* completely. It's rather weird to have a generic MIGRATEPAGE_UNMAP, documented to be returned from address-space callbacks, when it's only used for an internal helper. Let's start by having only a single "success" return value for migrate_folio_unmap() -- 0 -- by moving the "folio was already freed" check into the single caller. There is a remaining comment for PG_isolated, which we renamed to PG_movable_ops_isolated recently and forgot to update. While we might still run into that case with zsmalloc, it's something we want to get rid of soon. So let's just focus that optimization on real folios only for now by excluding movable_ops pages. Note that concurrent freeing can happen at any time and this "already freed" check is not relevant for correctness. But when I use IS_ENABLED(CONFIG_ZSMALLOC), page_movable_ops() cannot access zsmalloc_mops because zsmalloc_mops is in a module. To solve this problem, we define a set_movable_ops() interface to register and unregister offline_movable_ops / zsmalloc_movable_ops in mm/migrate.c, and call them at mm/balloon_compaction.c & mm/zsmalloc.c. Since offline_movable_ops / zsmalloc_movable_ops are always accessible, all #ifdef / #endif are removed in page_movable_ops(). Convert to page_has_movable_ops(). While at it, cleanup relevant code a bit. The data_race() in migrate_folio_unmap() is questionable: we already hold a page reference, and concurrent modifications can no longer happen (iow: __ClearPageMovable() no longer exists). Drop it for now, we'll rework page_has_movable_ops() soon either way to no longer rely on page->mapping. Wherever we cast from folio to page now is a clear sign that this code has to be decoupled. Previously, if __ClearPageMovable() were invoked on a page, this would cause __PageMovable() to return false, but due to the continued existence of page movable ops, PageMovable() would have returned true. With __ClearPageMovable() gone, the two are exactly equivalent. So we can replace PageMovable() checks by __PageMovable(). In fact, __PageMovable() cannot change until a page is freed, so we can turn some PageMovable() into sanity checks for __PageMovable(). Let's factor it out, simplifying the calling code. Before this change, we would have called flush_dcache_folio() also on movable_ops pages. As documented in Documentation/core-api/cachetlb.rst: "This routine need only be called for page cache pages which can potentially ever be mapped into the address space of a user process." So don't do it for movable_ops pages. If there would ever be such a movable_ops page user, it should do the flushing itself after performing the copy. Note that we can now change folio_mapping_flags() to folio_test_anon() to make it clearer, because movable_ops pages will never take that path. Add documentation and a comment that the remaining folio stuff (lock, refcount) will have to be reworked as well. While at it, convert the VM_BUG_ON() into a WARN_ON_ONCE() and handle it gracefully (relevant with further changes), and convert a WARN_ON_ONCE() into a VM_WARN_ON_ONCE_PAGE(). Note that we will leave anything that needs a rework (lock, refcount, ->lru) to be using folios for now: that perfectly highlights the problematic bits. For arrays with more than 16 entries, the old code would incorrectly advance the pages pointer by 16 words instead of 16 compat_uptr_t. This might be needed when 128-bit Page Table Descriptors are enabled for ARM. - "Always call constructor for kernel page tables" from Kevin Brodsky ensures that the ctor/dtor is always called for kernel pgtables, as it already is for user pgtables. Instead of flushing each address range in the provided iovec, we batch the flushing across all the iovec entries. The syscall's cost was approximately halved with a microbenchmark which was designed to load this particular operation. - "Track node vacancy to reduce worst case allocation counts" from Sidhartha Kumar makes the maple tree smarter about its node preallocation. stress-ng mmap performance increased by single-digit percentages and the amount of unnecessarily preallocated memory was dramaticelly reduced. - "mm/gup: Minor fix, cleanup and improvements" from Baoquan He removes a few unnecessary things which Baoquan noted when reading the code. - ""Enhance sysfs handling for memory hotplug in weighted interleave" from Rakie Kim "enhances the weighted interleave policy in the memory management subsystem by improving sysfs handling, fixing memory leaks, and introducing dynamic sysfs updates for memory hotplug support". As a consequence of the pagecache lookup path taking the folio lock this serializes against migration paths, so they can wait for each other. For the private_lock atomic case, a new BH_Migrate flag is introduced which enables the lookup to bail. This allows the critical region of the private_lock on the migration path to be reduced to the way it was before ebdf4de5642fb6 ("mm: migrate: fix reference check race between __find_get_block() and migration"), that is covering the count checks. 2025-03-17mm: convert folio_likely_mapped_shared() to folio_maybe_mapped_shared()David Hildenbrand1-4/+3
Let's reuse our new MM ownership tracking infrastructure for large folios to make folio_likely_mapped_shared() never return false negatives -- never indicating "not mapped shared" although the folio *is* mapped shared. With that, we can rename it to folio_maybe_mapped_shared() and get rid of the dependency on the mapcount of the first folio page. The semantics are now arguably clearer: no mixture of "false negatives" and "false positives", only the remaining possibility for "false positives". Thoroughly document the new semantics. We might now detect that a large folio is "maybe mapped shared" although it *no longer* is -- but once was. Now, if more than two MMs mapped a folio at the same time, and the MM mapping the folio exclusively at the end is not one tracked in the two folio MM slots, we will detect the folio as "maybe mapped shared". For anonymous folios, usually (except weird corner cases) all PTEs that target a "maybe mapped shared" folio are R/O. As soon as a child process would write to them (iow, actively use them), we would CoW and effectively replace these PTEs. Most cases (below) are not expected to really matter with large anonymous folios for this reason. Most importantly, there will be no change at all for: * small folios * hugetlb folios * PMD-mapped PMD-sized THPs (single mapping) This change has the potential to affect existing callers of folio_likely_mapped_shared() -> folio_maybe_mapped_shared(): (1) fs/proc/task_mmu.c: no change (hugetlb) (2) khugepaged counts PTEs that target shared folios towards max_ptes_shared (default: HPAGE_PMD_NR / 2), meaning we could skip a collapse where we would have previously collapsed. This only applies to anonymous folios and is not expected to matter in practice. Worth noting that this change sorts out case (A) documented in commit 1bafe96e89f0 ("mm/khugepaged: replace page_mapcount() check by folio_likely_mapped_shared()") by removing the possibility for "false negatives". (3) MADV_COLD / MADV_PAGEOUT / MADV_FREE will not try splitting PTE-mapped THPs that are considered shared but not fully covered by the requested range, consequently not processing them. PMD-mapped PMD-sized THP are not affected, or when all PTEs are covered. These functions are usually only called on anon/file folios that are exclusively mapped most of the time (no other file mappings or no fork()), so the "false negatives" are not expected to matter in practice. (4) mbind() / migrate_pages() / move_pages() will refuse to migrate shared folios unless MPOL_MF_MOVE_ALL is effective (requires CAP_SYS_NICE). We will now reject some folios that could be migrated. Similar to (3), especially with MPOL_MF_MOVE_ALL, so this is not expected to matter in practice. Note that cpuset_migrate_mm_workfn() calls do_migrate_pages() with MPOL_MF_MOVE_ALL. (5) NUMA hinting mm/migrate.c:migrate_misplaced_folio_prepare() will skip file folios that are probably shared libraries (-> "mapped shared" and executable). This check would have detected it as a shared library at some point (at least 3 MMs mapping it), so detecting it afterwards does not sound wrong (still a shared library). Not expected to matter. mm/memory.c:numa_migrate_check() will indicate TNF_SHARED in MAP_SHARED file mappings when encountering a shared folio. Similar reasoning, not expected to matter. mm/mprotect.c:change_pte_range() will skip folios detected as shared in CoW mappings. Similarly, this is not expected to matter in practice, but if it would ever be a problem we could relax that check a bit (e.g., basing it on the average page-mapcount in a folio), because it was only an optimization when many (e.g., 288) processes were mapping the same folios -- see commit 859d4adc3415 ("mm: numa: do not trap faults on shared data section pages.") (6) mm/rmap.c:folio_referenced_one() will skip exclusive swapbacked folios in dying processes. Applies to anonymous folios only. Without "false negatives", we'll now skip all actually shared ones. Skipping ones that are actually exclusive won't really matter, it's a pure optimization, and is not expected to matter in practice. In theory, one can detect the problematic scenario: folio_mapcount() > 0 and no folio MM slot is occupied ("state unknown"). One could reset the MM slots while doing an rmap walk, which migration / folio split already do when setting everything up. Further, when batching PTEs we might naturally learn about a owner (e.g., folio_mapcount() == nr_ptes) and could update the owner. However, we'll defer that until the scenarios where it would really matter are clear. 2025-03-16mm: use READ/WRITE_ONCE() for vma->vm_flags on migrate, mprotectLorenzo Stoakes1-1/+1
According to the syzbot report referenced here, it is possible to encounter a race between mprotect() writing to the vma->vm_flags field and migration checking whether the VMA is locked. There is no real problem with timing here per se, only that torn reads/writes may occur. Therefore, as a proximate fix, ensure both operations READ_ONCE() and WRITE_ONCE() to avoid this. This race is possible due to the ability to look up VMAs via the rmap, which migration does in this case, which takes no mmap or VMA lock and therefore does not preclude an operation to modify a VMA. When the final update of VMA flags is performed by mprotect, this will cause the rmap lock to be taken while the VMA is inserted on split/merge. However the means by which we perform splits/merges in the kernel is that we perform the split/merge operation on the VMA, acquiring/releasing locks as needed, and only then, after having done so, modifying fields. We should carefully examine and determine whether we can combine the two operations so as to avoid such races, and whether it might be possible to otherwise annotate these rmap field accesses. A shmem folio can be either in page cache or in swap cache, but not at the same time. Namely, once it is in swap cache, folio->mapping should be NULL, and the folio is no longer in a shmem mapping. In __folio_migrate_mapping(), to determine the number of xarray entries to update, folio_test_swapbacked() is used, but that conflates shmem in page cache case and shmem in swap cache case. It leads to xarray multi-index entry corruption, since it turns a sibling entry to a normal entry during xas_store() (see [1] for a userspace reproduction). Fix it by only using folio_test_swapcache() to determine whether xarray is storing swap cache entries or not to choose the right number of xarray entries to update. We replaced a simple put_page() by a putback_active_hugepage() call in commit 3aaa76e125c1 ("mm: migrate: hugetlb: putback destination hugepage to active list"), to set the "active" flag on the dst hugetlb folio. Nowadays, we decoupled the "active" list from the flag, by calling the flag "migratable". Calling "putback" on something that wasn't allocated is weird and not future proof, especially if we might reach that path when migration failed and we just want to free the freshly allocated hugetlb folio. Let's simply handle the migratable flag and the active list flag in move_hugetlb_state(), where we know that allocation succeeded and already handle the temporary flag; use a simple folio_put() to return our reference. This does not work if CONFIG_MEM_ALLOC_PROFILING_DEBUG=n because set_codetag_empty() becomes NOOP. Instead, let's simply swap codetag references so that the new page is referencing the old codetag and the old page is referencing the new codetag. This way accounting stays valid and the logic makes more sense. 2024-11-05mm/thp: fix deferred split unqueue naming and lockingHugh Dickins1-2/+2
Recent changes are putting more pressure on THP deferred split queues: under load revealing long-standing races, causing list_del corruptions, "Bad page state"s and worse (I keep BUGs in both of those, so usually don't get to see how badly they end up without). The relevant recent changes being 6.8's mTHP, 6.10's mTHP swapout, and 6.12's mTHP swapin, improved swap allocation, and underused THP splitting. Before fixing locking: rename misleading folio_undo_large_rmappable(), which does not undo large_rmappable, to folio_unqueue_deferred_split(), which is what it does. But that and its out-of-line __callee are mm internals of very limited usability: add comment and WARN_ON_ONCEs to check usage; and return a bool to say if a deferred split was unqueued, which can then be used in WARN_ON_ONCEs around safety checks (sparing callers the arcane conditionals in __folio_unqueue_deferred_split()). Just omit the folio_unqueue_deferred_split() from free_unref_folios(), all of whose callers now call it beforehand (and if any forget then bad_page() will tell) - except for its caller put_pages_list(), which itself no longer has any callers (and will be deleted separately). Swapout: mem_cgroup_swapout() has been resetting folio->memcg_data 0 without checking and unqueueing a THP folio from deferred split list; which is unfortunate, since the split_queue_lock depends on the memcg (when memcg is enabled); so swapout has been unqueueing such THPs later, when freeing the folio, using the pgdat's lock instead: potentially corrupting the memcg's list. __remove_mapping() has frozen refcount to 0 here, so no problem with calling folio_unqueue_deferred_split() before resetting memcg_data. That goes back to 5.4 commit 87eaceb3faa5 ("mm: thp: make deferred split shrinker memcg aware"): which included a check on swapcache before adding to deferred queue, but no check on deferred queue before adding THP to swapcache. That worked fine with the usual sequence of events in reclaim (though there were a couple of rare ways in which a THP on deferred queue could have been swapped out), but 6.12 commit dafff3f4c850 ("mm: split underused THPs") avoids splitting underused THPs in reclaim, which makes swapcache THPs on deferred queue commonplace. Keep the check on swapcache before adding to deferred queue? Yes: it is no longer essential, but preserves the existing behaviour, and is likely to be a worthwhile optimization (vmstat showed much more traffic on the queue under swapping load if the check was removed); update its comment. Memcg-v1 move (deprecated): mem_cgroup_move_account() has been changing folio->memcg_data without checking and unqueueing a THP folio from the deferred list, sometimes corrupting "from" memcg's list, like swapout. Refcount is non-zero here, so folio_unqueue_deferred_split() can only be used in a WARN_ON_ONCE to validate the fix, which must be done earlier: mem_cgroup_move_charge_pte_range() first try to split the THP (splitting of course unqueues), or skip it if that fails. Not ideal, but moving charge has been requested, and khugepaged should repair the THP later: nobody wants new custom unqueueing code just for this deprecated case. The 87eaceb3faa5 commit did have the code to move from one deferred list to another (but was not conscious of its unsafety while refcount non-0); but that was removed by 5.6 commit fac0516b5534 ("mm: thp: don't need care deferred split queue in memcg charge move path"), which argued that the existence of a PMD mapping guarantees that the THP cannot be on a deferred list. As above, false in rare cases, and now commonly false. Backport to 6.11 should be straightforward. Earlier backports must take care that other _deferred_list fixes and dependencies are included. There is not a strong case for backports, but they can fix cornercases. When numa balancing is enabled with demotion, vmscan will call migrate_pages when shrinking LRUs. migrate_pages will decrement the the node's isolated page count, leading to an imbalanced count when invoked from (MG)LRU code. The result is dmesg output like such: $ cat /proc/sys/vm/stat_refresh [77383.088417] vmstat_refresh: nr_isolated_anon -103212 [77383.088417] vmstat_refresh: nr_isolated_file -899642 This negative value may impact compaction and reclaim throttling. The following path produces the decrement: shrink_folio_list demote_folio_list migrate_pages migrate_pages_batch migrate_folio_move migrate_folio_done mod_node_page_state(-ve) <- decrement This path happens for SUCCESSFUL migrations, not failures. Typically callers to migrate_pages are required to handle putback/accounting for failures, but this is already handled in the shrink code. When accounting for migrations, instead do not decrement the count when the migration reason is MR_DEMOTION. As of v6.11, this demotion logic is the only source of MR_DEMOTION. random.h is not needed since commit 6c542ab75714 ("mm/demotion: build demotion targets based on explicit memory tiers"), all functions moved into memory-tiers. nsproxy.h is not needed since commit 228ebcbe634a ("Uninline find_task_by_xxx set of functions"), no nsproxy, we only call find_task_by_vpid() now. hugetlb_cgroup.h is not needed since commit ab5ac90aecf5 ("mm, hugetlb: do not rely on overcommit limit during migration"), move_hugetlb_state() is called and it belongs to hugetlb.h, which is already included. balloon_compaction.h, we have more general movable_operations for non-lru movable page migration, so it could be dropped. memremap.h, userfaultfd_k.h and oom.h are introduced for zone device page migration, but all functions are moved into migrate_device.c, so no needed anymore too.
